Job summary

Open Sky Community Services in Massachusetts seeks a Relief Counselor to support individuals facing mental health challenges and help them live independently. You will provide direct care, assist with daily living needs, and maintain a safe, supportive environment.

You will develop person-centered support plans, help clients access community resources or transportation, and document activities and progress with accuracy.

Qualifications

• High School Diploma or GED required.
• Valid Driver’s License with acceptable driving record.
• Reliability and willingness to learn; open to new opportunities.

Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
