Job summary

Open Sky Community Services is seeking Relief Counselors in Central Massachusetts to support individuals with mental health challenges, helping them live independently and pursue meaningful relationships. You will assist in creating person-centered plans, arrange resources or transportation, and help individuals build skills through engaging activities.

Documentation and collaboration with a trauma-informed, inclusive team are core parts of the role.

Qualifications

• Must have High School Diploma, GED or equivalent.
• Must have Valid Driver’s License and acceptable driving record.
• Reliability, willingness to learn, and being open to new opportunities.

Responsibilities

• Develop and implement individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
