Open Sky Community Services is seeking a Relief Counselor to join our team in Central Massachusetts. You will provide direct support to individuals facing mental health challenges, helping them live independently and pursue meaningful, inclusive lives while maintaining safety and dignity.

As part of the Relief Team, you will develop person-centered plans, assist with community resources and transportation, promote skills and social engagement, and complete required documentation to support each

Qualifications

• High School Diploma, GED or equivalent.
• Valid Driver’s License and acceptable driving record.
• Reliability, willingness to learn, and being open to new opportunities.

Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
