Open Sky Community Services is seeking a Relief Counselor in Massachusetts to support individuals facing mental health challenges, helping them live independently and pursue meaningful relationships. The role offers flexible scheduling and a chance to gain experience within a trauma-informed, inclusive organization.

Responsibilities include developing person-centered plans, assisting with community access and transportation, promoting skill development, and ensuring proper documentation.

Qualifications

• High School Diploma, GED or equivalent required.
• Valid Driver’s License and acceptable driving record required.
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
