Open Sky Community Services seeks a Relief Counselor to support individuals with mental health challenges to live independently and engage in their communities. The role emphasizes direct care, safety, and fostering meaningful relationships in a trauma-informed, inclusive environment.

You will develop person-centered plans, help access resources, and document progress while reinforcing wellness and autonomy for clients across Central Massachusetts.

Qualificações

• High School Diploma, GED or equivalent is required.
• Valid Driver’s License and acceptable driving record.
• Reliability, willingness to learn, and openness to new opportunities.

Responsabilidades

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
