Qualifications

• High School Diploma, GED or equivalent required.
• Valid Driver’s License with acceptable driving record.
• Reliable and willing to learn new opportunities.

Responsibilities

• Develop and implement individualized, person-centered support plans.
• Help individuals access community resources and provide transportation as needed.
• Implement activities that promote skill development and community engagement.
• Review and complete all required documentation.
