Job summary

Open Sky Community Services in Massachusetts is seeking Relief Counselors to support individuals facing mental health challenges, helping them live independently and engage meaningfully in the community.

You will develop person-centered plans, assist with transportation and access to resources, and document accurately, while benefiting from an organization that emphasizes trauma-informed care and strong supervision.

Qualifications

• High School Diploma, GED or equivalent required.
• Valid Driver’s License and acceptable driving record, required.
• Reliability, willingness to learn, and being open to new opportunities.

Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
