Open Sky Community Services is seeking a Relief Counselor to support individuals with mental health challenges, helping them live independently and engage with their communities. You’ll help develop person-centered plans, assist with daily living and transportation as needed, and ensure safe, supportive environments.

Qualifications

• Requires High School Diploma or GED.
• Valid Driver’s License and acceptable driving record.
• Reliability, willingness to learn, and openness to new opportunities.

Responsabilités

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
