Job summary

Open Sky Community Services Inc. seeks a Relief Counselor to support adults facing mental health challenges, helping them live independently and engage in meaningful activities. The role offers flexible scheduling and impactful direct care in a compassionate setting.

You will develop person-centered support plans, assist with accessing community resources or transportation, promote skill-building and relationships, and complete required documentation with attention to detail.

Qualifications

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License and acceptable driving record, required.
• RelIability, willingness to learn, and being open to new opportunities.

Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
