Job summary

Terros Health is seeking a Rehabilitation Specialist to join our interdisciplinary team at the Oak Recovery Health Center in Phoenix, AZ. This full-time role (40 hours/week) offers a robust benefits package and opportunities to support individuals with Serious Mental Illness or General Mental Health diagnoses under Court Ordered Treatment.

Typical schedule is Monday through Friday, 8:00am–5:00pm, at 3540 E. Baseline Rd, Unit #150.

Qualifications

• Experience providing rehabilitation services or supports for individuals with mental health diagnoses.
• Strong documentation and progress-tracking skills.
• Ability to collaborate with interdiscplinary teams and partners.

Responsibilities

• Assist clients with identifying treatment and recovery goals and provide supportive services to help clients reach employment, education, and community involvement goals.
• Coordinate rehabilitation-related services for clients served on the clinical team.
• Communicate and document client progress toward recovery.
• Embrace the recovery philosophy and promote a safe recovery environment.
• Use brief clinical interventions and supportive listening to engage clients.
• Engage and support clients' natural support systems.
• Work with Employment Specialists, RSA Vocational Rehabilitation Counselors, and community program providers to maintain employment, education, and activities.

Seeking a Rehabilitation Specialist to join our interdisciplinary team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Oak Recovery Health Center.

Full-Time Employment: 40 hours per week

Monday - Friday: 8:00am - 5:00pm

Duties Include

• Assist clients with identifying treatment and recovery goals, and provide supportive services to help clients reach their employment, education, and community involvement goals.
• Implement services identified in Individual Service Plans, and coordinate rehabilitation-related services for clients served on the clinical team.
• Communicate and document client progress toward their recovery.
• Embrace the recovery philosophy and promotes a safe recovery environment for clients.
• Use brief clinical interventions, Motivational Interviewing, and supportive listening skills to engage and support clients receiving services.
• Engage and provide direct support to clients' natural support systems.
• Work in partnership with co-located Employment Specialists and RSA Vocational Rehabilitation Counselors, community program providers, and other parties involved to assist clients in maintaining employment, education, and other meaningful daily activities.
