Pine Meadows Post Acute in Lexington, KY is seeking a Staff Physical Therapist to evaluate residents, develop personalized treatment plans, and support mobility goals. Qualifications

Licensed as a Physical Therapist in the state.
Must meet continuing education requirements per state practice.
Supervise PT assistants, aides, and students as needed.

Responsibilities

Evaluate residents to determine PT needs and rehab potential per physician orders.
Develop, implement, and update individualized treatment plans.
Assist nursing staff with training of Restorative Aides and supervise PTAs.
Communicate progress and goals to physicians, staff, residents, and families.
Document accurately and timely in medical records; ensure compliance.
Educate residents and families on home programs, safety, and fall prevention.
