Job summary

Lexington-Post-Acute is seeking a Staff Occupational Therapist to assess residents, develop individualized treatment plans, and guide a team of assistants. The role emphasizes collaboration with physicians, families, and staff to improve mobility and functional independence.

You will supervise COTA and aides, provide in-services, maintain compliant documentation, and participate in rehabilitation meetings to support resident care and outcomes.

Qualifications

• Bachelor’s degree in Occupational Therapy; Master’s or Doctorate preferred.
• Licensed in the state; CPR certification if required by licensure.
• Ability to supervise COTAs and aides.
• Proficient computer skills with Net Health and PointClickCare.

Responsibilities

• Evaluate residents to determine occupational therapy needs and rehab potential.
• Develop, implement, and update individualized treatment plans.
• Train Restorative Aides and supervise COTAs and aides.
• Communicate progress and discharge plans with physicians, residents, families.
