Job summary

Advocate Health is seeking a Nurse Aide to join the Rehabilitation Unit in Milwaukee. The role supports daily patient care, including bathing, feeding, mobility, and vital signs, with documentation in the EMR.

You will assist with admissions, transfers, discharges, and patient transitions under RN supervision. The position requires a completed high school education, a Nurse Aide certification in good standing, and BLS certification within 45 days.

Qualifications

• Completion of high school education or equivalent.
• Nurse Aide in good standing with state registry.
• BLS for Healthcare Providers certification within 45 days.
• Basic computer skills.

Responsibilities

• Assist RN with daily patient care including bathing, feeding, mobility and vitals; document data in EMR.
• Support admission, transfer, discharge processes; ensure room setup and patient belongings are secure.
• Perform rounding and communicate patient or family needs to RN; document observations.
• Support nutrition and skin integrity tasks; manage supplies like TED hose and IPC devices.
• Collect specimens and assist with point-of-care testing; follow site requirements.
• Follow infection prevention policies and proper PPE; maintain cleanliness.
• Coordinate tests and treatments; use EHR for orders and charges.
• Facilitate communication via multi-line phones and paging; troubleshoot equipment.
• Prepare chart forms for admissions/transfers/discharges; verify demographics.
• Maintain positive patient relations and age-appropriate care; observe growth and development.
