Job summary

PAM Health Rehabilitation Hospital of Jupiter is seeking a Clinical Navigator to be the face of the hospital, guiding patients and families through rehab journeys with support and motivation.

Under the Director of Business Development, you will cultivate referral relationships, manage the assigned territory, and perform patient assessments to streamline referrals and admissions. You will collaborate with admissions, nursing, and payer staff to improve the patient experience.

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Responsibilities

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Works collaboratively with Admissions Manager, admission staff, nursing and other internal and external staff to facilitate referrals and admissions.
• Provides feedback and recommendations for program development and quality improvement initiatives related to customer service, referral and admission processes, and stakeholder satisfaction.
• Develops and maintains excellent relationships with prospective patients, family members, physicians, ICU and floor nurses, discharge planners, case managers, and payer representatives.
