PAM Health Rehabilitation Hospital of Greater Indiana North seeks a Clinical Navigator to drive referral relationships, manage territory, and complete patient assessments. You are the face of the hospital, building trust with providers, payors, and families.

You will collaborate with the Director of Business Development, Admissions team, and clinical staff to optimize referrals and admissions while ensuring exceptional patient service throughout rehab stays.

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.
• Must have a minimum of two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.

Responsabilités

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Makes sales contacts and analyzes referral/admission data from territory.
• Collaborates with Admissions Manager, nursing staff, and other internal/external staff to facilitate referral conversion.
• Provides feedback and recommendations for program development and quality improvement in customer service, the referral and admission process, and patient/referral source satisfaction.
• Develops and maintains excellent relationships with stakeholders including prospective patient, family, physicians, ICU and floor nurses, discharge planners, case managers and payer representatives.
