Job summary

PAM Health Rehabilitation Hospital of Jupiter seeks a Clinical Navigator to drive referral development and patient access. You will cultivate relationships with physicians, discharge planners, and payer representatives to optimize admissions and ensure a smooth referral-to-admission process.

You will work under the direction of the Director of Business Development, analyzing referral data, coordinating with admissions teams, and contributing to program improvements while delivering

Qualifications

• Degree in a health-related field from an accredited college or university is preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Minimum of two years’ experience in the designated field, preferably with rehabilitation/LTACH and marketing experience.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and makes sales contacts.
• Analyzes referral and admission data to improve processes.
• Collaborates with Admissions Manager, nursing staff and payer representatives to facilitate admissions and ensure satisfaction.
• Develops and maintains relationships with stakeholders including patients, families, physicians, discharge planners and case managers.

Education

Degree in a health-related field

Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy
