Job summary

PAM Health in Missouri is seeking a Clinical Navigator to serve as the face of the hospital, cultivating referral relationships, managing an assigned territory, and completing patient assessments during rehab stays.

The role involves sales contacts, data analysis of referrals and admissions, and collaboration with admissions, nursing and other staff to facilitate referrals. Licensure in a health-related field and 2+ years in the field are required.

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Must have a minimum of two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.

Responsibilities

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Makes sales contacts and analyzes referral/admission data.
• Collaborates with Admissions Manager, nursing and other staff to facilitate referral and admission processes.
• Provides feedback and recommendations for program development and quality improvement related to customer service and patient satisfaction.
• Develops and maintains relationships with stakeholders including patients, family members, physicians, discharge planners, case managers and payer representatives.
