Job summary

PAM Health Rehabilitation Hospital of Greater Indiana North is seeking a Clinical Navigator to cultivate referral relationships, manage territory, and complete patient assessments within our facility. You will collaborate with the Director of Business Development and cross-functional teams to optimize admissions and referrals.

The role requires a health-field degree and active licensure in a qualifying clinical discipline, plus a minimum of two years’ experience in the licensed field; marketing

Qualifications

• Degree in a health-related field is required.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Minimum of two years’ experience in the designated field of license; rehab/LTACH marketing experience preferred.

Responsibilities

• Cultivates referral relationships with stakeholders.
• Manages assigned territory and completes patient assessments.
• Makes sales contacts and analyzes referral and admission data.
• Works with Admissions Manager and staff to facilitate referral conversion.
• Provides feedback and recommendations for program development and quality improvement.
