Job summary

You will work with a national rehabilitation leader to impact patient outcomes and community well‑being.

The role emphasizes collaboration across care teams, adherence to regulations, and active participation in discharge planning and documentation.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice.
• Active license required for the state if licensure applies.
• CCM or ACM certification required or must be obtained within two years.

Responsibilities

• Coordinate care with interdisciplinary team to plan discharge.
• Guide treatment plans based on patient needs and preferences.
• Coordinate with team to establish tentative discharge and contingency plans.
• Participate in planning for and executing patient discharge.
• Monitor patient experience and service quality, timeliness and payor expectations.
• Facilitate weekly team conferences and modify treatment plans as needed.
• Complete case management addendums and documentation in patient records.
