Job summary

Encompass Health in Glen Mills is seeking a Case Manager to coordinate care from admission to discharge, working with an interdisciplinary team to develop and adjust plans of care that optimize patient outcomes. This is a pivotal role at a leading national inpatient rehabilitation provider committed to compassionate, high‑quality care.

You will assess patient needs, arrange resources for timely discharges, monitor plan effectiveness, and participate in weekly conferences.

Qualifications

• Must hold a nursing degree or be a qualified health professional with a relevant license.
• CCM or ACM certification must be obtained within two years of entry or already held.

Responsibilities

• Coordinate patient care from admission to discharge with an interdisciplinary team.
• Develop and update plans of care to meet patient needs and payor requirements.
• Monitor and adjust discharge planning to ensure timely, safe transitions.
• Educate patients and families about rehabilitation goals and resources.
• Participate in case management meetings and maintain documentation.
• Ensure compliance with regulations and company policies.

Education

Bachelor's degree in Nursing or related field

CCM or ACM certification required or within two years
