Job summary

Encompass Health Corporation is seeking a Case Manager to coordinate patient care from admission through post-discharge. You will work with an interdisciplinary team to develop and adjust care plans, establish discharge plans, and educate patients and families on rehabilitation and the Case Manager role.

The role emphasizes regulatory compliance, documentation, and timely care decisions within a leading post-acute care provider known for recognizing employees and fostering growth.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice of one's discipline.
• Active license if licensure is required for the discipline.
• CCM® or ACM™ certification required or must be obtained within two years of entry.

Responsibilities

• Coordinate patient care from admission to post-discharge.
• Work with interdisciplinary team to develop and adjust care plans.
• Establish tentative discharge plans and contingency plans.
• Educate patient and family on rehabilitation and the Case Manager role.
• Maintain compliance with regulations and company policies.

Education

Associate Degree in Nursing (RN) or BSN preferred

Bachelor's degree for other professionals

Graduate degree preferred
