Job summary

Encompass Health Rehabilitation Hospital of Irmo is seeking a Case Manager to coordinate patient care from admission through post-discharge, working with an interdisciplinary team to develop and implement comprehensive plan-of-care decisions. You will help ensure timely discharges and optimize patient outcomes in a leading national inpatient rehabilitation network.

Qualifications

• RN licensure with BSN preferred; diploma acceptable only in states where that is the minimum

Responsibilities

• Work with interdisciplinary team to guide treatment plans based on patient needs and preferences.
• Coordinate tentative discharge plan and contingency plans with the team.
• Participate in planning for and the execution of patient discharge experience.
• Facilitate weekly team conferences and coordinate all treatment plan modifications.
• Complete case management addendums and all required documentation.
• Maintain knowledge of regulations, company policies, and department operations.
• Review case management reports and plan actions based on Key Care Indicators.
• Attend ACT meetings to identify trends and reduce ACTs.
• Meet with patient/family within 24 hours of admission; assess goals within 48 hours.
