Job summary

FreedomCare LLC is seeking a Case Resolution Specialist I to support patients and caregivers remotely, primarily in Pennsylvania, with occasional travel to a nearby hub. You will coordinate intake and onboarding, resolve Medicaid and enrollment issues, and act as a single point of contact for restoring homecare services.

Ideal candidates bring a passion for customer service, strong communication, and experience with CRMs like Salesforce.

Qualifications

Experience handling a high volume of inbound and outbound calls in a service environment.
Ability to clearly document case notes and statuses in a CRM.
Strong communication and problem-solving skills with a customer-first approach.

Responsibilities

Provide high-level customer service through one-call resolution.
Meet monthly metrics for enrolling patients and handling calls.
Be the primary contact for patients and caregivers restoring services and guiding enrollment.
Maintain timely email and voicemail responses within 1 business day.
Collaborate with Intake, Care Teams, and health plan portals to determine eligibility and obtain authorizations.
Document patient/caregiver interactions and case notes in systems.
