FreedomCare LLC in Pennsylvania is hiring a Case Resolution Specialist I for a remote role with occasional travel to the FreedomCare hub nearby. You will partner with patients and health plans to identify barriers to continuing services and help former patients restore homecare services, handling intake and onboarding for both patients and caregivers.

The role emphasizes one-call resolutions, timely responses, meeting enrollment targets, navigating e-Paces and health plan portals, and

Qualifications

Experience handling high-volume inbound and outbound calls.
Strong customer-service focus with empathy and active listening.
Ability to document case statuses clearly and timely.
CRM software experience, preferably Salesforce.
Excellent time management and multitasking abilities.
Strong verbal and written communication skills.

Responsabilités

Provide high-level customer service through one-call resolution.
Meet monthly metrics for starting patients on Care and call handling.
Serve as primary contact for patients and caregivers restoring services.
Maintain timely email, chat, and voicemail responses (1 business day).
Enroller 22 patients and caregivers onto the PA homecare program monthly.
Navigate systems (e-Paces, health plan portals) to determine eligibility and authorization.
