Chartiers Center is seeking a Registered Wellness Nurse to manage a caseload with complex health needs in a community-based integrated care setting.

You will build relationships, deliver wellness coaching, and coordinate transportation to appointments while maintaining compliant documentation and collaborating with a multidisciplinary team.

Qualifications

• RN certification from an accredited institution with PA licensure.
• Minimum of 3 years behavioral health experience.
• Valid driver's license required.

• Utilize wellness coaching to empower individuals to participate in managing their health and care.
• Deliver services in homes, community locations, healthcare facilities, or provider sites as needed.
• Coordinate transportation to medical appointments using agency vehicles when necessary.
• Review behavioral and physical health data to identify needs and develop wellness plans.
• Monitor progress and provide follow-up to promote adherence to treatment plans.
• Develop preventive wellness strategies focusing on nutrition, weight management, exercise, screenings, and tobacco cessation.
• Connect individuals with healthcare providers and community resources to improve health outcomes.
• Maintain timely documentation in accordance with agency, regulatory, and funding requirements.
• Provide consultation to colleagues regarding care and coordination of services.

This Registered Wellness Nurse is responsible for managing a caseload of individuals who have complex health and wellness needs, including those experiencing acute health events, unstable or progressive chronic medical conditions, barriers to accessing appropriate physical health, behavioral health, or substance use treatment services, and individuals who would benefit from health coaching, care coordination, and enhanced support in connecting with healthcare providers and community resources.

Job Duties:

• Build positive, supportive relationships with individuals and assist them in achieving their self-identified health and wellness goals.
• Utilize evidence-based wellness coaching practices to empower individuals to actively participate in managing their health and care.
• Deliver services in settings that best meet the individual's needs, including homes, community locations, healthcare facilities, and other provider sites.
• Coordinate and, when necessary, facilitate transportation to medical appointments using agency vehicles.
• Review and analyze behavioral health and physical health utilization data to identify needs and develop individualized wellness plans.
• Monitor progress and provide follow-up support to promote adherence to treatment recommendations and care plans.
• Develop and implement preventive wellness strategies focused on areas such as nutrition, weight management, exercise, health screenings, and tobacco cessation.
• Connect individuals with healthcare providers, community resources, and support services to improve overall health outcomes.
• Maintain accurate and timely documentation in accordance with agency, regulatory, and funding requirements.
• Provide consultation and support to colleagues regarding the care and coordination of services for individuals served.
• Participate in educational activities and group-based learning opportunities that promote healthy lifestyles, wellness, and disease prevention.
• Serve as a resource to individuals, families, and staff on topics related to health promotion, wellness, and care coordination.

Qualifications:

Registered Nurse (RN) Certification from an Accredited Institution with PA licensure with 3 years behavioral health experience

Valid driver's license required
