Job summary

Chartiers Center is seeking a Mobile Case Manager to provide in-home and community-based support for adults with serious mental illness and co-occurring conditions. You will assess needs, develop recovery-focused service plans, and connect clients with housing, health care, benefits, and employment resources.

You will collaborate with clinicians, family members, and community partners to ensure coordinated care, monitor progress, and advocate for services, all while maintaining timely

Qualifications

  • Associate’s degree or higher in human services or related field.
  • Experience in mental health or behavioral health services preferred.
  • Valid driver’s license and ability to travel in the community.

Responsibilities

  • Meet individuals in homes and in the community to provide support and service coordination.
  • Conduct comprehensive assessments to identify needs and recovery goals.
  • Develop and monitor individualized service plans promoting stability and independence.
  • Connect individuals with housing, healthcare, benefits, employment, and supports.
  • Collaborate with providers, families, and community agencies for coordinated care.
  • Maintain regular contact to monitor progress and identify emerging needs.
  • Advocate for clients and help navigate relevant systems and services.
  • Complete accurate documentation per agency and regulatory requirements.
  • Participate in on-call rotation, typically once per month.

As a Mobile Case Manager with Chartiers Center, you will have the opportunity to make a meaningful impact in the lives of adults living with serious mental illness and co-occurring conditions, including substance use disorders, and physical health challenges. In this dynamic, community-based role, you will build supportive relationships, connect individuals with essential resources, and serve as an advocate for their well-being and success.

Job Duties Include:
  • Meet with individuals in their homes and throughout the community to provide support, guidance, and service coordination.

  • Develop, implement, and monitor individualized service plans that promote stability, growth, and independence.
  • Connect individuals with critical resources, including housing, healthcare, behavioral health services, benefits, employment opportunities, and community supports.
  • Collaborate with healthcare providers, therapists, family members, and community agencies to ensure coordinated and effective care.
  • Maintain regular contact with individuals to monitor progress, address challenges, and identify emerging needs.
  • Serve as a strong advocate, helping individuals navigate systems and access the services they need to succeed.
  • Complete accurate and timely documentation in accordance with agency and regulatory requirements.
  • Participate in a shared on-call rotation, typically no more than once per month.

Minimum Qualifications:
  • Associate’s degree in human services and 3+ years of mental health experience.
  • Bachelor's degree in human services and 2 years of mental health experience OR
  • Master’s degree in human services with one year of experience
  • Valid Driver’s License Required
  • Ability to travel throughout the community
