• Associate's degree (or equivalent) with one-year behavioral health experience.
• OR High School Diploma/GED and two years behavioral health experience.
• Valid Driver’s License Required.

Responsibilities

• Assess consumer needs related to social rehabilitation using standardized assessment tools, direct observation, and individualized evaluation techniques.
• Design, coordinate, and facilitate therapeutic interventions, activities, and events that address identified consumer needs, with an emphasis on group-based programming.
• Conduct required assessments and evaluations utilizing program-specific and standardized tools to monitor consumer progress and outcomes.
• Collaborate in the development and implementation of individualized therapeutic and rehabilitative plans based on assessed consumer needs.
• Foster a positive, supportive therapeutic environment by serving as a role model, encouraging social interaction, and providing consistent support and reinforcement.
• Maintain accurate, timely, and complete documentation in accordance with program, agency, and regulatory requirements.

Education

Associate's degree / equivalent with 1 year behavioral health experience

High School Diploma / GED with 2 years behavioral health experience

Chartiers Center is a private, non-profit corporation funded by state, county, third party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program. As a Social Rehabilitation Assistant with Chartiers Center the core job duties include:
