Chartiers Center in Mt. Lebanon, PA is hiring an Intensive Mobile Case Manager for the Janus Program. You will join a multidisciplinary team delivering behavioral health services, assess participant needs, and develop person-centered service plans.

This full-time, Monday–Friday position offers competitive salary and benefits including medical, dental, vision, disability coverage and retirement plan, plus paid holidays and tuition reimbursement.

Qualifications

• Master’s degree in human services or related fields with 2 years of mental health experience
• Bachelor’s degree in human services or related fields and 3 years of mental health experience OR
• Associate's degree in human services or related fields and 4 years mental health experience

Responsibilities

• Coordinate and manage the delivery of behavioral health, medical social service and community support services.
• Assess participant needs and develop individualized, person-centered service plans.
• Meet with participants on a frequent basis to provide ongoing case management interventions and monitor participant progress.
• Maintain accurate and timely documentation in compliance with organizational and regulatory requirements.
• Participate in rotating on-call responsibilities to provide after hours support for Janus program participants.

Chartiers Center is a private, non-profit corporation funded by state, county, third party insurances and private funds. Janus Program is a new model of care that uniquely assists participants to liven on their own and learn desired skills.The Intensive Mobile Case Manager is a member of the multi-disciplinary team that works closely with program participants. Although we embrace a approach the Enhanced case manager provides personalized and specialized Service Coordination to participants of the Janus Program.

Minimum Qualifications:

• Master’s degree in human services or related fields with 2 years of mental health experience
