Job summary

Clarity Care in Northeast Wisconsin seeks an RN to provide clinical leadership and mentor residential caregiving teams. You will guide member health, support admissions, and coordinate with physicians to ensure quality care across homes.

The role emphasizes teaching, coaching and ensuring medication safety while meeting Wisconsin DHS requirements. Travel between homes is required, with a starting pay of $35/hour.

Qualifications

• RN license required in Wisconsin.
• Two years of experience with individuals with developmental or physical disabilities preferred.
• CBRF-approved instructor status in Medication Administration, First Aid, Fire Safety and Standard Precautions preferred.
• Knowledge of Wisconsin DHS requirements and agency policies.
• Strong communication, judgment and relationship-building skills.

Responsibilities

What You’ll Do:

Provide Clinical Leadership & Member Support:

• Assess Member health needs during admissions, hospital readmissions and changes in condition.
• Collaborate with residential leadership, physicians and care teams on individualized care plans.
• Communicate health concerns, respond to emergencies and coordinate home health or hospice services.

Teach, Mentor & Empower Caregiving Teams:

• Provide clinical direction, demonstrations, coaching and situation-specific staff training.
• Delegate and monitor medication-related duties in accordance with applicable regulations.
• Deliver required new-hire training through Clarity Care’s Training Institute.

Champion Medication Safety & Quality:

• Monitor medication administration and partner with Quality Assurance on safe, consistent practices.
• Support compliance with Wisconsin DHS requirements, nursing standards and agency policies.
• Identify opportunities to improve Member safety and care quality.

Support Employee Health & Safety:

• Assist with new-employee health screenings and occupational health activities.
• Administer, read and document two-step TB tests; perform COVID-19 testing and respirator fit testing as requested.

A compassionate, organized RN who enjoys clinical leadership, teaching and mentoring, with strong communication, judgment and relationship-building skills.

Education & Qualifications:

• Current Wisconsin Registered Nurse license required.
• Training and experience in personal care services or a related healthcare program.
• Understanding of Wisconsin DHS 13, 83, 88 and Chapters 50 and 51 requirements.
• Two years of experience with individuals with developmental or physical disabilities preferred.
• CBRF-approved instructor status in Medication Administration, First Aid, Fire Safety and Standard Precautions preferred.
• Working knowledge of Microsoft Word, Excel and Outlook.
• Valid Wisconsin driver’s license, proof of auto insurance and ability to meet agency driving standards.
• Frequent travel to residential homes throughout Northeast Wisconsin; required caregiver background checks.
