Job summary

Clarity Care, Inc. in Wisconsin is seeking an Activity Support Provider to assist with the Day Services Program. You will help members with daily living activities, plan engaging community and recreational activities, and support personal care needs.

No experience is required, and paid training is provided as we hire immediately to support individuals with varying abilities in our community. The role offers up to $18/hour, weekend premiums, and benefits that support a healthy, fulfilling life.

Qualifications

• Must possess a high school diploma/equivalent and/or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver’s license, and ability to meet agency driving standards. Must provide proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

Responsibilities

• Support members in community and recreational activities.
• Assist members with physical, social, and personal needs.
• Guide members in achieving personal goals and decision-making.
• Create fun and engaging activities tailored to member interests.
• Prepare and serve meals following dietary needs and preferences.

The Activity Support Provider assists with the day-to-day operations of the Day Services Program. They carry out responsibilities in the following functional areas: goal setting, independence, community involvement, consumer rights, personal cares, daily living activities, and regulatory compliance.
