Qualifications

• Active RN license in Wisconsin (or multi-state equivalent)
• Current BLS/CPR certification required on hire

• Experience in skilled nursing, sub-acute rehab, or acute care is valued; orientation provided

Key Responsibilities

As a vital clinical leader, the RN Registered Nurse oversees the following key areas:

• Comprehensive Clinical Oversight & Patient Assessment:

  • Perform thorough head-to-toe physical, cognitive, and psychosocial assessments upon admission, readmission, and routinely across shifts.

  • Continuously evaluate resident well-being, detect subtle physical or emotional status changes, and immediately report critical clinical findings to attending physicians and directors of nursing.

  • Manage complex clinical procedures, including advanced wound care, IV therapy administration, central line management, and tracheostomy care in accordance with Wisconsin scope of practice rules.

• Accurate Documentation, Medication & Care Planning:

  • Maintain precise, real-time clinical electronic health records (EHR) in strict compliance with state and federal long-term care regulations.

  • Transcribe physician orders accurately onto MARs/TARs, coordinate with pharmacy suppliers for timely medication deliveries, and safely administer complex pharmacological treatments.

  • Lead the development, periodic review, and revision of individualized resident care plans, ensuring alignment with interdisciplinary goals and physician directives.

• Unit Leadership, Supervision & Interdisciplinary Coordination:

  • Supervise, direct, and mentor LPNs and CNAs on your unit to ensure the highest standards of daily personal care, meal intake tracking, and hygiene are delivered with total dignity.

  • Collaborate closely with physical, occupational, and speech therapists, social workers, dietitians, and family members to ensure smooth care transitions and effective discharge planning.

• Emergency Response & Safety Advocacy:

  • Lead the clinical response during acute medical emergencies, initiating Basic Life Support (BLS) protocols and coordinating emergency medical transportation when needed.

  • Maintain strict adherence to infection control standards, HIPAA privacy laws, OSHA guidelines, and facility safety procedures to preserve a safe therapeutic environment.

Qualifications

To qualify for the role of RN Registered Nurse, candidates must possess:

• Licensure: An active, unrestricted RN Registered Nurse license in good standing in the state of Wisconsin (or multi-state NLC equivalent).

• Certifications: Current Basic Life Support (BLS) / CPR certification required upon hire.

• Clinical Judgment: Strong critical thinking, diagnostic assessment capabilities, and the ability to act decisively and calmly in a fast-paced healthcare setting.

• Experience: Skilled nursing, sub-acute rehabilitation, or acute care hospital experience is valued; however, high-potential new graduate RN Registered Nurse candidates are warmly encouraged to apply—we offer a comprehensive orientation and clinical preceptorship!
