BAYADA Home Health Care in Philadelphia, PA is seeking Transition Team Registered Nurses to support care transitions from hospital or HICU Health Services to at-home care. You will deliver high-quality care and train the BAYADA team in a collaborative, learning-focused environment.

Openings include full-time and part-time roles with day and overnight shifts, and a strong benefits package. Travel within Philadelphia County is required as part of routine coverage.

Qualifications

• Current license as a Registered Nurse.
• NICU, PICU, ICU, or home care trach/vent experience required.
• CPR certification current.
• Ability to travel throughout Philadelphia County.

Responsibilities

• Deliver quality care to clients on the Transition Team.

Job description

We have immediate opportunities on our Transition Team for Registered Nurses / RNs interested in helping infants, children, and adults receive complex care at home in Philadelphia, PA! This team will support the transition of care from the hospital or from our HICU® Health Services team to ongoing home care services. This role offers a unique opportunity to partner with CHOP and learn how to care for clients with rare and complex diseases. As part of the Transition Team, you will will be responsible for delivering quality care to clients and providing extensive training to the ongoing care team.

Present openings are overnights (Monday through Sunday nights) and day shifts (Saturday and Sunday days).

Qualifications for Transition Team Registered Nurses / RNs:

• Have a current license as a Registered Nurse / RN
• NICU, PICU, ICU, or home care trach/vent experience required
• Achieve a passing score on high-tech Adult and Pediatric simulations.
• Hold current CPR certification
• Ability to work in a teaching setting to prepare and educate the ongoing BAYADA care team
• Availability to commit to an ongoing, consistent schedule
• Ability to travel throughout Philadelphia County

RNs at BAYADA earn a salary of $30-$45 per hour.

Rate is based on qualifications and experience for the Transition Team role.

Transition Team rates: $40-$45

Standard rates: $30-$35
