BAYADA Home Health Care is hiring Transition Team RNs and LPNs to deliver high-quality pediatric home care. You will work with families to support the transition from hospital to home for infants and young children in Lehigh and Northampton counties.

The role offers competitive pay up to $45 per hour, a $1,000 sign-on bonus after completing 100 billable hours on a high-acuity trach/vent client, and referral bonuses up to $2,700. Flexible full-time and part-time schedules are available.

Qualifications

• Pediatric nursing experience preferred, especially in NICU/PICU/home care.

Responsibilities

• Provide skilled pediatric home health care to infants and young children.
• Collaborate with families and BAYADA care teams to ensure smooth transitions from hospital to home.
• Deliver hands-on training to caregivers and ongoing care teams.
• Maintain high-quality, one-on-one patient care in the home setting.

We have immediate openings on our Transition Team for RNs and LPNs passionate about caring for infants and young children as they transition from hospital to home in the Lehigh and Northampton County area.

In this role, you will:

• Work in a collaborative, team-based environment as an extension of our HICU® team
• Provide skilled, high-quality care to pediatric clients in the comfort of their homes
• Support families during the critical early stages of home care
• Deliver hands-on training to caregivers and ongoing care teams

Qualifications

• Active RN or LPN license
• Minimum of one year of verifiable nursing experience (pediatric experience preferred)
• Experience in NICU, PICU, ICU, or home care with trach/vent highly preferred
• Achieve a passing score on high-tech pediatric simulations
• Current CPR certification
• Ability to educate and support the ongoing BAYADA care team
• Commitment to a consistent schedule

Expectations

• Willingness to travel up to 1 hour within Lehigh and Northampton counties
• Availability for at least 2 weekend shifts per month
• Reliable and flexible to support team and client scheduling needs
