Job summary

BAYADA Home Health Care is seeking passionate pediatric nurses for their Transition Team in Allentown, PA. We have immediate openings on our Transition Team for RNs and LPNs passionate about caring for infants and young children as they transition from hospital to home in the Lehigh and Northampton County area.

In this role, you will:

• Work in a collaborative, team-based environment as an extension of our HICU® team
• Provide skilled, high-quality care to pediatric clients in the comfort of their homes
• Support families during the critical early stages of home care
• Deliver hands‑on training to caregivers and ongoing care teams

Compensation & Schedule

• Availability for at least 2 weekend shifts per month
• Reliable and flexible to support team and client scheduling needs
