Qualifications

• Registered Nurse license in the practicing state; CPR certification.
• Associate degree in nursing or equivalent; 1+ year in skilled/sub-acute/memory care.
• English fluency for effective communication with residents and staff.

Responsibilities

• Medication supervision/administration per Federal & State rules and company guidelines.
• Manage admissions including assessments and documentation.
• Develop and update acute and long-term care plans or service plans.
• Track clinical data (e.g., wounds, infection control) under the Director of Nursing.
• Handle discharges, assessments, and clinical documentation as needed.
• Provide health consultations and notify physicians/families of changes.
• Respond promptly to emergencies and uphold confidentiality.
• Coordinate with all departments; participate in safety procedures.
• Offer educational sessions for residents and staff.

• Responsible for medication supervision/administration in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
• May be responsible for the admissions of new residents to include all admission assessments and documentation.
• Responsible for completing acute and long-term care plans or service plans.
• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the Director of Nursing.
• May be responsible for discharges, assessments, and clinical documentation as indicated.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
• Respond quickly to all emergencies.
• Maintain close communication with all departments and staff members.
• Maintain confidentiality of residents’ and prospective residents’ information.
• Understand role in the Safety & Disaster Plan.
• Provide educational meetings for residents and staff.

Level of Formal Education: An Associate’s Degree or equivalent from a two year college or technical school or Two years related experience and/or training equivalent combination of education and applicable experience.

Special Certifications: Must have a Registered Nurse’s License current, with no judgments against it, in the state in which he/she is practicing and must be CPR certified
