Job summary

Ascension is hiring an RN to provide dedicated clinical care in a school setting at Washington Township School System Crooked Creek Elementary in Indianapolis, IN. The role involves one-on-one nursing care for a student with a tracheostomy, with a schedule of Monday–Friday, 7am–3pm (37.5 hours).

Responsibilities include implementing care plans, coordinating with the care team, and ensuring safety and high-quality outcomes throughout the school day.

Qualifications

• BLS Provider required within 1 month of hire; American Heart Association or American Red Cross accepted.
• RN credentialed from Indiana Board of Nursing; valid state license in applicable state.

How you’ll make an impact in this role

• Implement and adapt personalized care plans by tracking, recording, and communicating vital health changes to guarantee seamless, high-quality patient outcomes.
• Coordinate the multidisciplinary care team to align physicians, specialists, and school staff, ensuring the student receives fully integrated and well-supported care.
• Execute physician and nursing orders with precision and empathy, maintaining strict adherence to clinical safety protocols.
• Manage proactive transition and discharge planning alongside the healthcare team to set the student up for long-term health and academic success.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required. Valid state license required specific to the state in which the associate works.

Education

• Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
