Polaris Healthcare and Rehabilitation Center is seeking an experienced Registered Nurse (RN) Supervisor to lead and mentor our nursing team in Milford, Delaware. You will ensure high-quality resident care, regulatory readiness, and effective coordination with physicians and families.

The role requires an active Delaware RN license and 2–3 years of supervisory experience, with familiarity in PCC documentation a plus.

Qualifications

• 2-3 years of RN supervisor experience.
• Current Delaware RN license.
• Knowledge of PCC nursing documentation preferred.

Responsibilities

• Promote regulatory compliance through long-term care regulations and survey readiness.
• Assist nursing team in providing resident care when needed.
• Supervise and evaluate resident care throughout the shift.
• Ensure staffing changes align with budgeted plan and address concerns.
• Identify learning needs and coordinate with Staff Development and leaders.
• Mentor and coach staff as needed.
• Round on units and respond to issues promptly.
• Communicate effectively and ensure respectful service to residents and families.
• Interact with residents' families to address questions and concerns.
• Transcribe doctor orders timely and accurately.
• Oversee administration of IVs and other meds.
• Assist with admissions/discharges of residents.
• Uphold safety and policies, and respond to emergencies.

Essential Functions

• Promote regulatory compliance through demonstrating working knowledge of long-term care regulations and participating in efforts towards consistent survey readiness
• Assist the nursing team in providing resident care when professional nursing skills/judgment is needed
• Continually supervise and evaluate resident care throughout the shift
• Ensure any staffing changes are in alignment with budgeted staffing plan and address any staffing concerns as they arise
• Identify learning needs of staff and participate in educational opportunities in collaboration with the Staff Development Coordinator and nursing leaders
• Provide mentoring and coaching to staff when needed
• Round on each nursing unit and respond to issues as needed/requested
• Demonstrate effective communication skills and positive service excellence behaviors when dealing with all customers
• Responsible for interacting directly with residents' families to answer their questions and ease their concerns
• Ensure and/or assist with transcribing all doctor orders timely and accurately
• Oversees and/or assists with administering IVs and other medications
• Assist with or complete admissions/discharging of the residents
• Ensure safety of residents, customers and facility through awareness of and conscientious application of policies and procedures and emergency responses

Qualifications

• 2-3 years of previous Registered RN Supervisor experience recommended
• Current Delaware RN license
• Knowledge of Point Click Care (PCC) nursing documentation program preferred
