Job summary

Elara Caring is seeking a Registered Nurse focused on Start of Care visits in Mentor, Painesville, Chardon, Ashtabula, Geneva and surrounding communities. You will assess patients and guide families to understand what comes next, laying the foundation for ongoing care.

The role centers on Start of Care assessments, coordinating with physicians, documenting thoroughly, and ensuring a smooth transition into home health. Ohio RN license and 1 year of clinical nursing experience required.

Qualifications

• Current RN license valid in Ohio.
• Graduation from an accredited nursing program (certificate, diploma, associate, or bachelor’s).
• At least 1 year of nursing experience in a clinical care setting.
• Home health experience preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients.
• Must possess a valid driver’s license or have reliable transportation.

Responsibilities

• Complete comprehensive Start of Care assessments to identify patient needs and establish a foundation for individualized care.
• Complete ongoing assessments and provide nursing care as assigned.
• Provide care according to the plan of care and physician orders.
• Monitor patient conditions, communicate changes, and take appropriate action.
• Help patients and families understand their care, safety needs, and when to seek assistance.
• Coordinate with physicians and the care team to support a smooth transition into home health.
• Maintain accurate clinical documentation and protect patient and staff privacy.
• Support safe, high-quality, continuous patient care.

What You’ll Need

• Current RN license valid in Ohio.
• Graduation from an accredited certificate, diploma, associate, or bachelor’s nursing program.
• At least 1 year of nursing experience in a clinical care setting .
• Home health experience preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
