Job summary

Elara Caring is hiring a Registered Nurse Home Health to serve Ashtabula, Lake, and Geauga Counties in Ohio. You will assess patients, follow physician orders, and coordinate care with the team to deliver quality outcomes.

Qualifications

Responsibilities

• Assess patients to determine care needs and plan.
• Complete initial and ongoing patient assessments.
• Provide nursing care per plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on care and safety.
• Support patient transfers and equipment handling.
• Contribute to safe, high-quality, continuous patient care.

As a Registered Nurse Home Health ,you’llcontribute to our success in the following ways:

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
