Job summary

Limitlessli is seeking a skilled Registered Nurse Case Manager to administer skilled nursing care in patients’ homes across Indiana. The role requires evaluating nursing needs, initiating and revising the plan of care, and coordinating services with the care team.

You will deliver services with specialized nursing skills, document care, and support patients and families, while maintaining compliance with agency policies and regulatory requirements.

Qualifications

  • RN licensure required.
  • Two years clinical nursing experience, preferably in home health.
  • Ability to work independently and coordinate care across settings.

Responsibilities

  • Provides services in accordance with the plan of care.
  • Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
  • Initiates the plan of care and necessary revisions.
  • Provides those services requiring substantial specialized nursing skills.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
  • Coordinates services.
  • Informs personnel of changes in the condition and needs of the patient.
  • Counsels the patient and family/significant others in meeting nursing and related needs.
  • Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.
  • Determines the amount and type of nursing needed by each individual patient.
  • Supervises and teaches other nursing personnel.
  • Conducts patient care conferences on patients assigned to his/her care.
  • Participates in peer review and Quality Assessment and Performance Improvement as assigned.
  • Gives total patient care as needed; Takes on-call duty nights, weekends and holidays, as assigned.
  • Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
  • Appropriately utilizes ICD-10 codes.
  • Performs other related duties as assigned.

Key Responsibilities

Education & Experience:
  • Graduate of an accredited nursing school. BSN preferred.
  • Minimum of two (2) years clinical nursing experience, preferably in home health/home care setting.
  • RN licensure and ability to obtain additional states of licensure.
  • Experience working with medically fragile clients, addressing medical, social and behavioral components of care.

Physical Requirements:
  • Sedentary – ability to remain in a stationary position for extended periods of time.
  • Ability to move, transport, or position: up to 50 pounds
  • Ability to move or traverse about in offices and/or client homes, including ascending and descending stairs.
  • Ability to communicate effectively and clearly with others to exchange information.
Travel Requirements:
  • Regular travel on a daily or weekly basis required, even in inclement weather
