Job summary

Select Home Health & Hospice is seeking a skilled and compassionate Registered Nurse to provide in-home nursing care. This role ensures patients receive high-quality, personalized care while promoting independence and well‑being.

The ideal candidate graduated from an accredited nursing program, holds an Indiana RN license, has at least 1 year of nursing experience (home health preferred), a valid driver’s license, and TB testing is up to date.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current Registered Nurse license in Indiana.
• Minimum 1 year nursing experience in hospital, facility, or home health.
• Valid driver's license, reliable auto insurance, and personal transportation.
• TB test current.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete required documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications, disease processes, and in-home care.
• Coordinate patient services and supervise LPNs/HHAs as needed.
• Communicate changes in patient condition to physicians and care teams.
• Maintain compliance with regulations and agency policies.

Physical Requirements

• Ability to sit, stand, and drive for extended periods while providing in‑home care.
• Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
• Regular use of a tablet for documentation, communication, and training.
