Job summary

Human Touch Home Health in Maryland is seeking an experienced Home Health Registered Nurse (RN) to join our growing team serving Prince George's County and surrounding communities. You'll provide personalized, one-on-one skilled nursing care at patients' homes to support recovery, chronic condition management, and independence.

The role emphasizes compassionate patient relationships, clear documentation, and collaboration with physicians and the care team.

Qualifications

• Active Maryland RN license required.
• Valid driver’s license and reliable transportation.
• Strong clinical assessment, critical thinking, and documentation skills.
• Home health experience preferred but not required.
• OASIS experience preferred.
• Experience with WellSky Home Health is a plus.

Responsibilities

• Perform comprehensive nursing assessments and skilled visits.
• Complete SOC, ROC, Recertification, and Discharge assessments.
• Develop and implement individualized plans of care.
• Provide skilled nursing services including medication management, wound care, disease management, IV therapy as appropriate, and patient education.
• Monitor patient progress and communicate changes with physicians and the interdisciplinary care team.
• Coordinate care with therapists, social workers, and home health aides.
• Complete timely and accurate clinical documentation using WellSky.
• Educate patients and caregivers to promote safety, independence, and improved health outcomes.
