Job summary

Behavioral Health Network, Inc. is seeking a Registered Nurse or Licensed Practical Nurse to join the PACT team in Western Massachusetts.

This part-time, 20 hours/week position offers a flexible schedule and opportunities for professional growth within a large behavioral health organization. The role involves coordinating medical and psychiatric care, maintaining MAP compliance, and providing nursing services across PACT locations.

Qualifications

  • RN or LPN licensure required or in process.
  • MAP certification within introductory period.
  • Minimum 6 months in Human Service field preferred.
  • Valid driver's license and reliable vehicle available for all working hours.

Responsibilities

  • Provide indirect and direct services to individuals in the PACT program.
  • Coordinate medical services and care planning with the team.
  • Manage pharmaceuticals and medical supplies under supervision.
  • Schedule and administer medical assessments and referrals.
  • Maintain shift logs and ensure coverage across PACT locations.

What You'll Do
  • As the Registered Nurse/Licensed Practical Nurse, you will provide indirect and direct services to individuals served within the PACT program.
  • This role collaborates with and provides medical services/coordination to facilitate the care planning process and overall provision of services to the individuals served.
  • Ensures all staff are following MAP Policies and Procedures and follows these procedures.
  • Completes health and medication-related documents as individually indicated and in accordance with MAP regulations.
  • Under the direction of the team Psychiatrist, Nurse Manager, and in collaboration with other Registered Nurses on the team, manages pharmaceuticals and medical supplies.
  • Coordinates, schedules, and administers medical assessments of individual's physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinates psychiatric treatment with medical treatment.
  • Ensures change of shift log is completed each shift and that change of shift occurs each shift with oncoming staff.
  • Provides safe and timely transportation in own vehicle, as needed for medical appointments, recreational activities, shopping, errands, etc.
  • Provides ongoing information, periodic reviews, and documentation to support treatment and Treatment Plan goals and objectives.
  • Maintains OSHA guidelines.
  • Provides coverage at all PACT locations as needed.
  • Completes individually assigned program-specific responsibilities.
Who You Are
  • Bachelor's Degree in related field preferred.
  • Registered Nurse (RN) licensure or Licensed Practical Nurse (LPN) certification required.
  • Must obtain and maintain MAP certification within introductory period of employment.
  • Minimum 6 months in Human Service field preferred.
  • Valid driver's license and reliable vehicle available for all working hours to transport individuals served required.
