Job summary

Behavioral Health Network (BHN) in Massachusetts seeks a Licensed Practical Nurse (LPN) to join its PACT team in Worcester. You will provide direct nursing care in homes and community settings, administer medications, educate clients and families, and coordinate with a multidisciplinary team to support recovery and independent living.

Requirements include a valid MA LPN license, a high school diploma or GED, and strong communication skills.

Qualifications

  • High school diploma or GED required
  • Minimum 1 year nursing experience preferred, psychiatric/behavioral health exposure a plus

Responsibilities

  • Participate in data collection related to psychiatric, medical, and functional status of individuals served on the PACT team
  • Provide direct nursing care within scope of practice, including psychiatric and medical medications in-office and in community settings
  • Educate clients and families on mental illness and co-occurring conditions under RN/NP guidance
  • Support medication adherence strategies, including injectable meds, blister packs, and home delivery as needed
  • Document care in the record and communicate with the multidisciplinary team at daily meetings and shift reports
  • Maintain confidentiality per 42 CFR
  • Provide health assessments and monitor chronic conditions in coordination with PCP/specialists
  • Prepare nursing assessments and progress notes per policy
  • Monitor medication logs and use EMR/staff logs for clinical updates
  • Serve as liaison to hospitals and medical facilities for physical/behavioral health needs
  • Participate in 24/7 PACT crisis intervention and community outreach visits
  • Provide in-service training and pursue ongoing staff development

What You Will Do
  • Participate in data collection related to the psychiatric, medical, and functional status and needs of individuals served on the PACT team
  • Provide direct nursing care within scope of practice, including administering and monitoring psychiatric and medical medications, both in‑office and in community settings (homes, shelters, and other locations)
  • Educate individuals served and their families on mental illness, co‑occurring medical conditions, treatment, and prognosis, reinforcing teaching under the direction of a Registered Nurse or Nurse Practitioner
  • Support medication adherence strategies, including injectable medication administration, blister‑packing, and home delivery of medications as needed
  • Thoroughly and accurately document all aspects of care in the individual's record, communicating pertinent information to the multidisciplinary team at daily organizational meetings and shift report
  • Maintain strict confidentiality of individuals served information in accordance with 42 CFR
  • Provide health assessments and monitor chronic medical conditions for individuals served, coordinating with primary care and specialty providers as needed
  • Prepare and submit nursing assessments and progress notes in accordance with organizational and regulatory policy
  • Monitor medication logs for accuracy and use EMR/staff logs to convey pertinent clinical information
  • Serve as liaison to area hospitals, emergency departments, and medical facilities, and act as a resource for individuals served and their families regarding physical and behavioral health needs
  • Participate in crisis intervention and outreach as part of the PACT team's 24/7 availability, including community‑based visits as scheduled
  • Provide ongoing in‑service training and consultation to staff, and participate in continuing education and staff development activities
Who You Are
  • High School Diploma or GED required
  • Current Massachusetts Board of Registration license as a Licensed Practical Nurse
  • Minimum one year of nursing experience preferred; experience with psychiatric or behavioral health populations a plus
  • Skilled in applying the nursing process to individuals served and their families, including those with serious mental illness and co‑occurring substance use or medical conditions
  • Comfortable providing services in community settings, including individuals' homes and other non‑clinical environments
  • Able to collaborate with and appropriately delegate to other caregivers within a multidisciplinary team
  • Knowledgeable in utilization/review standards, quality assurance, and applicable regulatory standards
  • Able to understand, empathize, and work effectively with individuals experiencing serious mental illness, including those with co‑occurring disorders
  • Working knowledge of the ethics and legal parameters of mental health and substance use treatment
  • Valid driver's license required
  • Excellent verbal and written communication skills
