Job summary

Behavioral Health Network, Inc. is seeking a Clinical Supervisor for the PACT Forensic Services team in Western Massachusetts.

You will supervise PACT staff, lead care coordination and treatment planning, and deliver hands-on clinical services across client homes, hospitals, and community settings. Requirements include a Master’s degree in Social Work or Psychology, active licensure or eligibility, MAP certification within six months, and a minimum of six months in human services.

Qualifications

• Master's Degree in Social Work or Psychology required.
• Independent licensure or licensure eligible required.
• MAP certification within introductory period (6 months) required.
• Minimum of six months experience in human services preferred.
• Valid driver's license and reliable vehicle required.

Responsibilities

• Coordinate clinical care and treatment planning for individuals served and involved family members and providers.
• Oversee risk plans and crisis plans to ensure safety of individuals served.
• Provide assessment and treatment for substance use, connecting to AA/NA/Detox/Methadone/Suboxone resources.
• Deliver services in client homes, hospitals, and other community settings.
• Complete progress notes and required documents by end of each shift.

What You Will Do:

• Assist with coordination of clinical care and treatment planning for individuals served, including all pertinent family members, community, and residential providers as appropriate
• Oversee and complete risk plans and crisis plans as required by supervisor, providing comprehensive risk/crisis evaluation and taking appropriate actions to keep individuals served and others safe
• Provide assessment and treatment of individuals with substance abuse/dependence, including facilitating dual recovery groups, connecting individuals to resources such as AA, NA, Detox, and Methadone/Suboxone treatment, and liaising with community agencies and families
• Maintain flexibility in providing services in settings such as client homes, hospitals, and other community settings
• Assist and monitor activities of daily living, encouraging independence, competency, and decision-making
• Provide outreach services to individuals served on a frequent basis as required by supervisor
• Act as primary implementer of strategies to meet Treatment Plan goals and assist with the Daily Organizational Meeting (DOM)
• Complete progress notes for all individuals by the end of each shift and complete program-specific and time-sensitive documents, such as Incident Reports and Medication Occurrence Reports, in a timely manner
• Complete health and medication-related documents in accordance with MAP regulations
• Complete initial assessments, treatment plans, treatment plan reviews, financial management plans, and assessments to assigned caseload, serving as primary case manager as required by supervisor
• Maintain direct services productivity as established by the Program and ensure assigned individuals served make positive change on the outcome measure used by the PACT Model

Who You Are:

• Master's Degree in Social Work or Psychology required
• Independent licensure or licensure eligible required
• Minimum of six months experience in human services preferred
• Must obtain and maintain MAP certification within introductory period of employment (6 months)
• Demonstrates willingness and ability to treat challenging and diverse client populations, including a forensic population
• Demonstrates working knowledge of community resources and how to help clients access them
• Valid driver's license and reliable vehicle available for all working hours to transport individuals served required
• Excellent verbal and written communication skills and commitment to empowerment, growth, human rights, and community integration of individuals served

Salary Range: $78,000-$84,500
