Job summary

JourneyCare Hospice in Chicago is seeking a compassionate Hospice RN Case Manager to provide patient-centered, end-of-life care with manageable caseloads and strong interdisciplinary support.

You will manage a dedicated hospice caseload, perform assessments, coordinate with physicians, and support families, with a Monday–Friday schedule and one weekend rotation every eight weeks.

Qualifications

• Graduate of an accredited RN program.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills.
• Valid driver’s license and proof of insurance.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed.
• Provide end-of-life support, including family education and pronouncement.
• Supervise Home Health Aides in compliance with regulations.

• $80,000 – $97,000 annually (based on experience)
