JourneyCare Hospice is seeking a Hospice RN to join our team. New competitive salaries, immediate opening available, generous time off packages, 401K + match and so much more!

HOURS: Monday-Friday 8:30am-5pm and One Weekend Every 12th Week (Saturday and Sunday)

TERRITORY: Bull Valley, Johnsburg, IL. Salary: $80,000-$97,000 Annually - Offer Based on Years of Experience

Qualifications

• Licensed as a registered nurse in the state of practice.
• Current CPR and First Aid certifications.
• Knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of the nursing process.
• Obtain data from patient physical, psychological, social, and spiritual factors impacting needs and coordinate intervention with the interdisciplinary team.
• Initiate communication with the attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, reports and timely care plans.
• Supervise and document delivery of care by Home Health Aide as required by guidelines.

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
