Job summary

BAYADA Hospice in Concord, NH is seeking a compassionate RN to join our team on a full-time basis. The role involves making home visits to clients in designated geographic territories and providing palliative care to help clients remain at home.

Prior home care or hospice experience is preferred but not required; we provide onboarding and training, and BAYADA emphasizes support, growth, and recognition for our staff.

Qualifications

• Current NH RN license required.
• Minimum of one year clinical (medical/surgical) experience.
• Home care/hospice experience preferred but not required.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medications, wound care, treatments and comfort measures.
• Monitor clients' conditions and report changes to supervisor.
• Follow doctors' orders and document actions.
• Perform client assessments as needed.

BAYADA Hospiceis looking for a compassionate and dedicated Full Time Registered Nurse (RN) to join our team in the Hospice office, located in Concord, NH. This office services hospice clients on a per visit basis in Merrimack, Belknap and Hillsborough counties.

Prior home care and/or hospice experience strongly preferred, but not required. As a hospice nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a Hospice RN - Registered Nurse include

• Making home visits to clients in designated geographic territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Occasional on call after hours and/or weekends

Qualificationsfor a Hospice RN - Registered Nurse include

• A current license as a Registered Nurse (RN) in NH.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care and/or hospice experience strongly preferred, but not required.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Base Pay, depending upon qualifications and subject to negotiation:

$42-$46 per hour
