Job summary

BAYADA Home Health Care, Norwich, VT is seeking a compassionate, full-time Registered Nurse (RN) to join BAYADA Hospice in our hospice office. You will provide patient-centered care, perform home visits, and support clients in Windsor, Orange, and Caledonia counties with no weekends and no on-call duties.

Minimum 1 year of clinical experience preferred; VT/NH RN license required. Strong EMR skills and willingness to travel to cases are needed.

Qualifications

• Current RN license valid in VT and NH.
• Minimum 1 year of clinical experience; hospice or home care preferred.
• Solid computer skills and EMR experience.

Responsibilities

• Make home visits to clients in designated Vermont counties.
• Administer medications, wound care, treatments, and comfort care.
• Monitor clients and report changes to the Clinical/Client Services Manager.
• Follow up on doctors' orders and document appropriately.
• Perform client assessments as needed.

Full time Hospice Case Manager Opportunity!

BAYADA Hospice is looking for compassionate and dedicated Full Time Registered Nurses (RN) to join our team in the Hospice office, located in Norwich, VT. This office services hospice clients on a per visit basis in Windsor, Orange, and Caledonia counties in VT and some of Grafton County in NH.

• NO Weekends, NO On Call!
• Weekly Pay
• Full time hours 8:30-5 Mon-Fri

Prior home care and/or hospice experience strongly preferred, but not required. As a hospice nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a Hospice RN - Registered Nurse include:

• Making home visits to clients in designated geographic territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Occasional on call after hours and/or weekends

Qualifications for a Hospice RN - Registered Nurse include:

• A current license as a Registered Nurse (RN) in VT and NH.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care and/or hospice experience strongly preferred, but not required.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Hour pay rate negotiable based on experience: $45-$55/hour
