Transitions Hospice LLC in Illinois is seeking a Night Nurse to join our overnight care team. You will provide professional nursing care, assess and respond to patient needs, and collaborate with the interdisciplinary team to maintain comfort and safety.

This role requires an active RN license in Illinois, graduation from an accredited nursing program, and ability to work independently during night hours. We offer rotating weekends, competitive pay, and comprehensive benefits.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice or home health preferred.

Responsabilités

• Delivering professional nursing care across the full nursing process within your scope of practice.
• Conducting thorough patient and family assessments and contributing to individualized care plans.
• Completing all visit types as needed to meet overnight patient and family needs.
• Administering medications and treatments per physician orders.
• Identifying nursing diagnoses from health assessment data and responding with sound clinical judgment.
• Recording accurate, timely clinical notes including pain and symptom management outcomes.
• Communicating changes in patient condition to physicians and obtaining orders as required.
• Coordinating with community health partners to support continuity of care.
• Ensuring equipment and necessary services are in place for patients.
• Maintaining and growing your clinical skills through continuing education and in-service participation.

Coverage Territory: Sangamon, Cass, Morgan, Mason, Menard and surrounding areas

Schedule: 4 - 10 hour shifts a week w/rotating weekends

Pay Range: $80,000 - $90,000 a year

What You'll Do

Requirements

What You Bring

• Active RN license in the state of employment
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a related setting preferred
• Comfort and composure working independently during overnight hours
• Strong assessment skills and sound clinical judgment in high-pressure moments
• A calm, reassuring presence for patients and families during nighttime distress
• Excellent documentation habits and communication skills
• Valid driver's license and willingness to travel locally
